We are going to re-floor the house. We want a couple of "floor" plugins. Do I need a permit to do this?

Yes, just pull a home owners permit.

Any electrical addition or change in your home, technically requires a permit.

Just remember, if you spill water in a floor plug bad things can happen. I don't recommend floor plugs flush with the floor. There are bases you can get that have gasket around them and are raised off the floor by a couple of inches. The plugs are actually mounted 90 degrees to the floor. The base is a commercial or industrial unit.
